Three years ago, FIP Your School Ohio launched with the goal of advancing the use of research-based formative instructional practices across the state. We want to hear specific stories involving your students or your personal practice. How has FIP impacted the way you teach, learn, and grow? What impact have formative instructional practices had on the students that step into your classroom?
On June 16th, we will be setting up a FIP Video Lounge at the Ohio Educators Connect for Success Conference in Columbus, Ohio. When you visit the video lounge, you’ll have the opportunity to share your FIP story on camera. This is your chance to tell your story, share your experiences, and celebrate your successes! We will also be giving an iPad Mini to one lucky educator that stops by the FIP Video Lounge.

What to Expect: We want you to hear your FIP story, but we won’t keep you from the conference experience. The FIP Video Lounge will be open from 1:00PM – 4:00PM and each filming session will last approximately five minutes.

What to Wear:
Don’t be afraid to be on camera! We’ll make sure our production team captures your best side, with professional lighting and equipment. Come to the conference dressed to impress; we’ll take care of the rest. We recommend wearing flat colors and simple textures with minimal logos or brand marks as possible. School apparel or logos are acceptable.
Where to Go
We will be capturing stories and collecting submissions on Monday afternoon, starting at 1:00PM and going through the afternoon. You will find us in Clark located nearby the escalator on the second floor, just steps away from many of the sessions you will be attending that afternoon. One of our staff members will be at a table outside the room to answer questions, prepare you for the brief video interview, and enter you in the iPad Mini raffle.

RAFFLE RULES AND REGULATIONS
No Purchase or Payment of Any Kind is Necessary to Enter or Win
This raffle is sponsored by Battelle for Kids (“Sponsoring Organizations”). By participating in this raffle, entrants agree to abide by all rules and regulations. Entrants understand that results of the raffle, as determined by the sponsoring organizations, are final in all respects.
To be eligible to win the iPad Mini, an individual must share a story about implementing formative instructional practices (FIP) via video on June 16, 2014 at the Connect for Success Conference held in Columbus, Ohio. Employees and relatives of the Ohio Department of Education or Battelle for Kids are not eligible to participate in the raffle. Sponsoring organizations reserve the right to verify the eligibility of each entrant. Entrants will have the option to sign a release allowing their likeness to be used for educational and promotional purposes, but such a release form is not a requirement for eligibility.
The winner will be randomly selected from a pool of all eligible entrants. The winner will be selected and notified within one week of the raffle. Winners will receive email notification and must provide a valid mailing address to receive the iPad Mini. If a potential prize winner cannot be reached by the sponsoring organizations within fifteen (15) days using the contact information provided at the time of entry, that prize winner shall forfeit the prize.
Limitations of Liability
Prize is non-transferable. No substitution or cash equivalent of prizes is permitted. Sponsoring organizations and their respective parent, affiliate and subsidiary companies, agents, and representatives are not responsible for any typographical or other errors in the offer or administration of the raffle.
By entering the raffle, each entrant agrees to indemnify, release and hold harmless sponsor and its parent, affiliate and subsidiary companies, and all their respective officers, directors, employees, representatives and agents from any liability, damages, losses or injury resulting in whole or in part, directly or indirectly, from that entrant’s participation in the raffle and the acceptance, use or misuse of any prize that may be won.
